Looking at your pictures, I agree with most of the comments so far.

One thing that was not mentioned that I think can make a big difference sometimes is the use of a breast strap or “bandeau” that is placed over the upper pole of the implants to provide pressure.

This would be worn 24 hours a day, if possible until the desired position of the implant is achieved. This will not alleviate all of the issues, but the constant pressure can help with repositioning the implants somewhat by stretching the capsule and allowing the implants to settle more and to a lower position.

This may help a bit with the implants sitting so high. Ideally, it would have been better to have done this early on after surgery, but after 2 months it still may be helpful. This will not help the appearance of the scars or the asymmetry, but if the implants settle more, the infra-mammary scars will not be as easily seen.

I dont think that you are going to get much of an improvement just by waiting, in other words, time is not going to make much of a difference on its own. When you loose 130 pounds and a significant amount of volume in the breasts it is important for the mastopexy to incorporate **precise** removal of the excess skin, particularly in the lateral direction towards the axilla. That being said, as mentioned by others, the implants are sitting too high and need to be lowered, again in approximately 6-8 months utilizing a capsulotomy.

This time, the nipple areolas’ can be raised to a better position. Also by curving the lateral incision towards the axilla, some of that redundant puckering could be removed, but again I would give it at least six months before attempting the revision. (William F. You are still a bit early after surgery, but I think that there are some issues that are resulting in an unsatisfactory result so far. The implants are a bit high, and widely separated. The scars are also suboptimal.

The most important part of plastic surgery is to wait 6 months before deciding anything. Judging from your pictures, i would guess that the scars will smooth out (very good chance), the nipples will probably change position, but may not be exactly where you want them to be, and the unevenness should also improve.

There is no need to worry, but in my opinion you will need a revision. It appears that you had a vertical scar lift, and some unevenness of the lower part of the breast usually is visible but settles out over time. Usually. Not always. But this excess and uneven skin can be revised.

I do not do vertical scar lifts, but many surgeons get excellent results with them, but they do take some time to settle. In order to avoid the scar in the inframammary crease, the vertical lift results in some irregularities that are supposed to smooth out. IF they don’t, a revision should fix it.

Your implants appear to be too high, and after two months I think they are going to remain too high. Some upper breast swelling for a couple of weeks is normal, taking some weeks for an implant to “expand” and “relax” into the lower part of the breast in a patient with tighter skin (which is not your case) is normal, BUT an implant that is too high is an implant that is too high.

The only things I know of that will cause this degree of distortion in position at 2 months are either significant capsular contractures (scar tissue squeezing the implants up high on the chest) OR the implants having been placed too high to begin with.in my experience, they are not coming down on their own. You should review the situation with your surgeon, and if not satisfied you should get a second opinion from another board-certified plastic surgeon.

But revision surgery should be posponed for a couple more months at least until the tissues have a chance to settle down. As mentioned, augmentation and major lifts performed at the same time are tricky, and there can be a very high revision rate, and patients need to be warned of this. (Robert M. While a minor revision can be done after breast augmentation with a breast lift, 2 months after the procedure is too early. Wait at least 6 months to allow for scar softening and skin stretching before judging the final results. In most cases, things improve and nothing needs to be done. (Karol A.
