When a patient comes in saying they want breast augmentation but want to look matural, I often tell them they look natural already, but they just don’t like it. What they usually mean is that they don’t want to look obviously unnatural.

The general philosophy and approach to naturalness (or minimal unnaturalness) is to try and hide the implant as much as possible behind the nautral breast tissue, and to keep the proportion of implant to natural breast tissue as low as possible.

The key, of course, is to do all this and still produce the enlargement that makes the operation worth it to the patient. If you think about putting in a golfball size implant in the breast, the breast will look and feel pretty much like a natural breast, but it would produce much enlargement.

If you put a softball size implant in the breast, the breast will be noticeably enlarged but will act and feel more like the implant. Most patients with even the smallest amount of breast tissue can accept some implant that will satisfy the above approach.

However, it might mean choosing a very small implant that gives only a modest amount of augmentation. For example, if your fiance has only abouot 100-150cc of breast tissue with a diameter of 10 cm., she might only be able to be matched with a 150cc. implant.

If she wants something larger, she will have to accept becoming more and more unnatural with more and more of the characteristics of the implant chosen. Since there are more complications and side effects with relatively larger implants, silicone implants under the muscle becomes more and more reasonable. The most natural results are when moderate size implants are placed. In very thin patients, it is probably best to place them in a submuscular pocket to give more soft tissue coverage, but you can obtain very natural results.

  • Submuscular vs. subglandular placement of implants

A lot of women with a small frame and tight skin choose to have breast augmentation. If done conservatively one can expect a nice and natural looking result.

I would recommend silicone implants for natural appearance and feel and caution you(her) against a large implant. There is a lot to consider. I recommend that she seek consultations with a few surgeons certified by the American Board of Plastic Surgery.

There is an important component of planning a most natural breast augmentation that involves matching the patient’s proportions and chest wall measurements/soft tissue characteristics with the appropriate sized implant.

This process is key to assuring a natural ”fit” for the implant to minimize undesirable over-stretching of tissues or an overly “fake” look with too large an implant. Conversely, too small an implant may fail to achieve a satisfactory change and can be disappointing to the patient.

I recommend using relatively small, round, smooth walled high profile implants placed under the muscle. Saline implants are fine in this setting, and can be put in through a very small incision. Silicone implants are also fine, but with a slightly longer scar. I would aim for a large A cup.

Thin tight skin with little breast tissue is a situation in which it is more difficult to look natural, if you mean that the implant is not detectable. However, patients such as this can be some of the happiest patients, because modest improvement is so significant for them. My brief advice would be to consider relatively small volume implants, placed subpectoral. The implants should be as wide as her natural breast area, which usually equates to a low or moderate profile style. I would encourage her not to try to achieve a full C-cup or larger postop size. Small B-cup may be natural appearing for her and minimize stretching her thin tissue.

For woman with very little breast tissue, breast augmentation can have a dramatic impact.

However, each woman’s breasts size, skin availability, skin tone, and overall body frame impact the choice of implant and the placement of the implant.

For women with very small breasts, the implant will be supplying the majority of the breast volume.

As you can imagine, this means that the implant needs to be concealed as much as possible.

I recommend placement underneath the pectoralis muscle. This is to soften the palpability of the implant as well as the visibility.

By placing the implant under the muscle, it reduces the likelihood of seeing rippling or distortions in the implant. In addition, many women with smaller breasts opt for silicone implant.

Because the implant is providing the majority of volume, the “feel” of the implant is very important. Silicone implants simply have a more natural feel than saline implants.

It is important to review all the pros and cons of silicone before this decision, and your plastic surgeon can help you with this. Also, be aware that silicone natural looking implants are not approved for patients under 22 years of age. Lastly, but in my opinion most importantly – implant size. The selection of an implant is based on several factors, but it is important to choose a size which fits your body.

This is determined by the width of the breast, the laxity of the skin, and the desired increase in size. Attempting to enlarge the breasts outside of what your body can handle usually leads to problems down the road and dissatisfaction in the long run. Many thin patients with little breast tissue successfully undergo breast augmentation everyday; however, when there is little breast tissue there are several things you need to consider. When it comes to the size of the implant, it is best to select a size that is suitable for her frame. A large implant can cause stretch marks and have a “fake” appearance. Another thing to consider is rippling. When saline breast augmentation patients have very little breast tissue, they may experience what is known as rippling. This is when the edges of the implant are not completely smooth and can be seen on the breast.

No patient is too small to gain a benefit from breast augmentation. To achieve a natural result when one has very little of her own tissue may require a submuscular gel implant of a modest size. I would expect her final cup size to be at most between a B and C. Patients with little breast tissue and subcutaneous fat will get nice results from natural looking implants but I feel that silicone gel is vastly superior to saline in this situation. With silicone gel, I’m not convinced that going under the muscle is necessarily aesthetically better although I will get some argument from other plastic surgeons on that opinion.
