The implant should be equal to or less than the width of your natural breast with a volume sufficient to achieve your goals. What most women really want to know is whether saline or silicone is “better.” In my office, I present both options for patients to feel and to get a sense of the weight.

By far the most obvious difference is in the feel. The viscosity of silicone is so much higher that silicone implants have a very natural feel.

In addition saline implants are a little heavier than the equivalent silicone implants. When in the breast, the silicone implants tend to have less problems with rippling.

I would suggest finding pictures of looks you would like to achieve. Depending on your internal anatomy, I usually place moderate to moderate plus profile implants submuscularly to achieve a more natural look.

High profile implants are used to achieve more projection. Sizers are used in the operating room to determine size and volume. The good news is that there are a lot of choices. The bad news is that all the choices can be confusing. Your doctor should thoroughly discuss with you the pros and cons of the different types of implants. Not only do you have to choose between silicone and saline but there are different dimensions as well. In the end you get a highly customized result.

Make sure your doctor has sizers in the office for you to try on in a bra. This is done after your doctor determines what range of implant sizes are appropriate for the dimensions of your body. Although there are many factors in making a nice breast augmentation, the type of implant is important. Over the last year I have been using a brand new implant which is more cohesive or “gummy bear” like.

It seems to hold its shape better and still feels very much like a natural breast. They are made by Sientra. If you imagine the implant being sliced in half with a bread knife, it would sit there like a gummy bear and not ooze out.

There are many choices for implants. They can be saline or cohesive silicone gel. Cohesive implants can be round or teardrop shaped and the surface can be smooth or textured.

The choice of implant depends on your existing shape and the type of breast you are trying to achieve. So there may be a best implant for you but it may not be the best implant for the next person.
