Fixing uneven breasts without general anesthesia?
Uneven breasts without general anesthesia
Mar 29, 2019
Hi and thank you for your question.  Anesthesia is something that scares a lot of people so you are definitely not alone in having this fear.  First thing to consider is what your breasts need.  Some things you can do under local anesthesia but many options also require anesthesia.  That being said, I use a sedation anesthesia for almost all of my cases including all my breast surgeries.  Sedation is different than a general anesthesia because you are sleeping but you are still brea…
Does Ibuprofen Increase Bleeding Risk after breast augmentation?
Ibuprofen
Mar 28, 2019
You are correct in that there are a few studies saying that ibuprofen is safe to use before and after surgery.  Some surgeons are okay with their patients taking ibuprofen and others are not because the mechanism of action inhibits platelets which stop bleeding.  It definitely makes a difference during surgery if a patient has been on ibuprofen but ultimately it’s up to the surgeon what he/she prefers.  I personally recommend against taking these types of medications.  In general, I woul…
What is meant by “upper pole fullness”? Do mine have upper pole fullness? photo)
Upper pole fullness
Mar 28, 2019
Hi and thanks for your question!  Upper pole fullness just means fullness in the top half of the breast.  It is usually the first thing to go with age or after pregnancy/breastfeeding.  Adding upper pole fullness is also a spectrum where mild to moderate amount can still be very natural.  Once you get to a very high amount of upper pole fullness it can start to look more round and fake.  What that looks like also depends on a lot of factors such as implant placement, size, shape, and how …
Do I have scleral show? Does it seem like something is wrong or I just have bigger eyes naturally? W
Scleral show
Mar 27, 2019
Yes it does appear that your lower eyelid has some laxity that may be causing your scleral show.  This is the most likely diagnosis but it should really be confirmed by an in person consultation.  There are various reasons why someone can have scleral show so you have to be sure you are correcting the actual cause.  If it is laxity in your lower eyelid the procedure of choice would be a canthopexy vs canthoplasty.  Good luck!
Should I get saline or silicone? Saline seem more safe, but my concern is that they won’t feel natur
Saline vs Silicone
Mar 26, 2019
Hi and thanks for your question!  In my opinion, silicone implants are vastly superior to saline implants.  Silicone implants are softer, more natural feeling, more natural appearing, have less risk for rippling, and are equally safe as saline implants.  Saline implants need to be overfilled which makes them feel much more like a hard water ballon than a breast.  Good luck!
Can (TIVA) Total IV Anesthesia be used for Full Face/Neck Lift?
TIVA anesthesia
Feb 23, 2019
I use TIVA for almost all of my surgeries including facelifts.  I do think it is the better option because patients wake up so much easier and faster from it.  My patients actually walk themselves from the operating room to recovery room, and then again from the recovery room to their ride outside.  A full face lift surgery can vary depending on what kind of technique is used but it probably ranges between 3 and 5 hours.
My PS said he can fix my breasts by opening the pockets up more laterally and putting in an internal br
Breast implant revision
Feb 23, 2019
Hi and thank you for your question.  I know this is probably not something you want to hear but in my opinion, your breast implants are too wide for your chest frame.  I would seriously consider a more narrow implant.  Moving them laterally may solve your cleavage problem but you may end up with another problem and that is too much “side boob.”  It can get in the way and a lot of people don’t like that look.  You just don’t want to replace one problem for a different problem.  Good luck!
Do I need a breast lift or can I get good results with just implants?
Breast lift vs implants
Feb 23, 2019
You are in a situation where many people often find themselves.  In a general sense, the choices are a breast lift, a breast lift with implants, and implants alone.  If you are happy with your size and don’t want to go any bigger then you should really be talking about a breast lift alone.  A breast lift will give you a much perkier breast but it comes at a cost.  There is always more scar when it comes to lifting anything and there is a risk to breastfeeding down the road.  Fortu…
I want large, perky breasts that are still proportional to my body. Will I need a lift?
Perky breasts
Jan 6, 2019
Hi and thanks for your question!  Based on your pictures I would not recommend a lift in you.  Just breast implants alone will give you fuller breasts.  Breast lifting surgery is only for people with what is called breast ptosis where the breast tissue sags but also the nipple starts to descend and start to point downward.  In your case your nipple is in a good position and does not need to be lifted.  Good luck!
Rounded nose tip? Is my nose too big for my face?
Rhinoplasty
Jan 6, 2019
Hi and thanks for your question!  It does appear that you would be a good candidate for a rhinoplasty.  RIght now your nose overall is short (in the up and down dimension) which accentuates your bulbous tip.  Refining your tip to make it more feminine.  Adding to the top part of the nose, called the dorsum, will elongate your nose and make it look longer and slimmer instead of shorter and fatter.  Best to go see a surgeon and discuss!  Good luck!
Can a canthopexy produce a lateral canthal tilt?
Canthopexy/canthoplasty
Jan 5, 2019
To answer your question, canthopexy/canthoplasty will pull the outer corner of the eye further out to the side and generally up to create the canthal tilt that you are referring to.  It’s hard to say without actual pictures of your eyes but if you have scleral show and a negative or neutral canthal tilt, then you may benefit from a lid tightening procedure such as a canthopexy/canthoplasty.  Best to go seek an in person consultation.  Good luck!
Can I have rhinoplasty if I’ve had an adenoidectomy?
Adenoidectomy and rhinoplasty
Jan 5, 2019
Adenoidectomy should not preclude you from having a rhinoplasty.  They are different parts of your anatomy and should really not affect one another.  That being said, your rhinitis might be something that you should look into as rhinoplasty can cause prolonged congestion and rhinitis type symptoms.  Good luck!
Would just a rhinoplasty fix the asymmetry in my face?
Facial asymmetry
Dec 29, 2018
Hi and thank you for your question and pictures!  I would suggest going the filler route for you at least to start.  Your bump is not really a bump on your nose but rather a deficiency in the radix which is the point at which your nose comes off your forehead.  It is lower than the ideal place so it makes it look like a bump.  Raising that point on your face with either filler or surgery would make your nose straight and harmonious with the rest of your face. As far as your left/right side…
What is wrong with my eyes and what can be done to fix them?
Eyes
Dec 23, 2018
Hi, it’s unfortunately impossible to tell based on your photo because your camera was too close to your face and it’s making you look a little up.  I would recommend reposting with a picture that is taken from further away and then either crop out areas or insert black boxes to cover.  Good luck!
Is incognito breast surgery possible with smaller, “soft” tissued breasts?
Undetectable breast implants
Dec 23, 2018
Hi and thanks for your question!  Breast implants are made to be soft and natural appearing.  That being said, I would say most people can’t tell by feel if someone has breast implants.  On the otherhand, a plastic surgeon is always going to be able to tell if someone has implants.  It all depends on who is feeling them.  By your pictures, you’d be a great candidate for breast implants.  Unfortunately it’s not possible to have them be undetectable by anyone.  Hope this helps and good luck!
