It appears you possess a fair amount of natural breast tissue. You have to ask yourself would you like to be a cup size larger by having an breast implant placed. My opinion is to go with the lift and no breast implant. You can always place an implant later if you are unhappy with the size. There are significantly less risks when a lift is performed without breast implants.

Can areolas be reduced and lowered?

In my opinion, I would redo your surgery. I recommend a breast lift with areola reduction, scar revision and possibly a small breast implant. This would improve all your scars, lift the breasts, and make them more perky. This approach would eliminate the problem with the areolar being exposed above your swim top.

If your RA is well controlled and your PCP provides the green light, I see no reason you cannot have the procedure at a surgery center. You may need to be off your meds for a short period of time. Best of luck.

Your photos reveal that the nipple is well above the breast fold. In breasts with your nipple location a breast lift is not needed. Implants alone will provide a very nice result.

I no longer use Responsive. I have been using Soft Touch/Cohesive exclusively. Personally, I like the idea that if the shell of the implant becomes defective the gel won't leak out. I have been using Soft Touch/Cohesive since they first became available in 2017. I have not had to perform a revision on one of these patients yet.

How would you give me a more natural-looking breast augmentation?

The easiest and fastest way to achieve your goal photograph is with breast implants. Fat transfer would take multiple sessions. Each session would increase your current breast 1 cup size. In the long run implants would be a lot less expensive.

Liposuction would definitely make your figure more curvy. The fat obtained could be transferred into the buttock and hips. The fat transfer would enhance the figure and provide a more feminine silhouette.

The photos reveal a Tuberous breast deformity. Your best option is bilateral breast augmentation, correction of nipple herniation and correction of tuberous appearance. I believe a circumareolar incision would be beneficial to correct the nipple protrusion and areola reduction. Consult with a plastic surgeon in your town.

Your best option is correction of Tuberous breast appearance, bilateral breast augmentation and correction of the nipple herniation with a circumareolar incision. I’m doing subfascial and want to do saline. Do you think I’ll have enough to avoid ripples?

It appears you have a fair amount of breast tissue to place an implant subfascial. I use this approach often in the right candidate. I am hesitant to recommend this approach with saline implants. Initially everything will look and feel great. However, I have found over time the implants will ripple more in the cleavage area. If you want saline implants I would suggest a submuscular placement.

Could I have a desirable outcome with a breast fat transfer without a lift?

Scars are a common concern many women have about undergoing a breast lift. In your case fat transfer alone will not provide an aesthetically pleasing breast appearance. I would consider a breast lift with or without additional fat transfer to provide additional upper pole fullness.

Am I considered to have slight Tuberous Breast Deformity with “severe” asymmetry?

You characteristics of a tuberous breast. The left breast is smaller, more conical, tight high breast fold, relatively narrow breast width.and herniation of the nipple areolar region. The left breast is larger, ptotic, with large areola but not truly tuberous. Your appearance can be significantly improved with surgery. Find an experienced breast surgeon in your area.

Do I have tubular breasts or are they just saggy and will implants underneath help?

First off your breasts do not look that bad. You have a fair amount of breast tissue. You do not need breast implants. The only way to improve the appearance and shape of your breasts would require incisions and subsequent scarring. Usually the scars are not that bad. Consult with a plastic surgeon to discuss your options.
