Your breast width is the main factor in choosing implant size. Your surgeon is right, going smaller than breast width does not give you the best result and also has the risk of implant rotation if the pocket is larger. The key to answer this question is how much of your own breast tissue you have. In cases of enough breast tissue (which the cut off may vary from surgeon to surgeon), the answer is yes. It can be done as a redo of the Wise pattern breast reduction or in some cases with smaller amount of extra tissue, minimal extra skin and good skin tone, liposuction could be an option too.
