I commonly use either BellaDerm (acellular human dermis) or Galatea (synthetic surgical mesh) in complex revision cases where I need to reconstruct the "floor" upon which a breast implant sits.

Breast asymmetry is extremely common and there are many techniques available that allow us to improve symmetry. For minor asymmetry, differences in implant pocket dissection and the use of slightly different sized implants can be very helpful. In cases of more severe asymmetry, completely different surgical procedures can be performed on each breast. For example, one breast may require only an implant while the other breast may require both an implant and a lift.

Capsular contracture is a well known complication of breast augmentation surgery. While the cause of capsular contracture is not completely known, there are several things that can be done in an attempt to decrease the risk. Bottoming out of breast implants is the result of a lack of support in the vicinity of the "floor" upon which the implants sit. This floor consists of the lower pole breast skin and soft tissue and the inframammary fold attachments. If the integrity of any of these structures is compromised, bottoming out can occur. I have had significant success reconstructing the lower pole of the breast and the inframammary fold using both Alloderm and Galatea.

There are many different ways to remove "belly fat." The first is diet and exercise, the second is liposuction, the third is a tummy tuck and in the case of intra abdominal fat, the fourth is diet modification and tightening of the anterior abdominal wall. In order to determine exactly which procedure is best for a patient, an in person consultation with a board certified plastic surgeon is required.

The studies performed by BLT, the manufacturer of Emsculpt, for FDA clearance, are all based on 4 treatments, performed over an approximately 2 week period. I have seen great results after 4 treatments. However, in my experience, I have found that the best results are seen after 6 treatments, all performed within 3 weeks, two times per week, no more frequently than every 48 hours.
