I think part of your problem is that you have skin that has lost its elasticity. And although a forearm lift will moderately improve that, some of it will definitely return because surgery does not change the intrinsic nature of the skin. The picture you supplied is not enough to give you a complete answer. Usually saddle nose deformities are much more extensive than what you see on that one less than ideal picture. If anything you look like you have something called an “inverted V deformity” which can occur in some post rhinoplasty patients who had a big hump removed. I don’t know if your nasal asymmetry is due to an accident or the accident may have contributed to an already existing asymmetry. Almost everybody has some degree of asymmetry. You are right about not having the absolute typical asian nose but again there are many variations within any population. I would agree with you that you need a hump removal but I cannot assess your skin thickness on a photograph. If you truly have very thick skin, which I doubt, you are most likely going to have moderately thick skin, you have to be careful because reducing the internal structures of the nose (the cartilages) especially of the tip area do not do well with reduction.
