One of the drawbacks of the trans-axillary technique, if it is performed without an endoscope, is that it can be difficult to get a good release of the pocket inferiorly.  This can lead to a high-riding implant.
Try the bandeau, but if it doesn't start dropping by 3-4 months out, you might have to consider re-adjusting the pocket surgically.
What can be done with this extra skin?

Hi.  It looks as though your symmastia revision is holding up fine, but you have recurrent laxity of just the medial infra-mammary skin.  Although it's a little hard to tell in the photos, it may benefit from re-tightening the skin, using the medial part of your breast lift incision.  It might require extending your scar, though.  Return to your surgeon to ask.

Hi.  It looks like you have normal veins together with fairly translucent skin.  If they bother you, and you don't want to simply use a cosmetic cover-up, you could have the veins treated with laser.  See a Board-certified plastic surgeon or dermatologist with laser experience in your area.

As far as I can tell from you photos, I think a short-scar breast lift would really add to the result, lifting the nipple to a more youthful position.  I'd be worried that after an augmentation alone, the nipple would still be low, and the breast somewhat saggy.  If you are not sure, you can always do the lift at a second stage.

Hi.  Looking at your photos, it seems like you don't have that much tissue laxity.  So, I don't think you need a tummy tuck - liposuction of the flanks and abdomen would, IMO, be the better choice.Of course, an in-person exam is needed to confirm all this.Best wishes

Hi.  Your scar pattern is actually not a "keloid", but what is called a "wide-spread scar".  This is where the scar widens out over time, either due to tension on the skin, or from simply having thin, stretchy tissues.If you downsize and re-do some of the lift, the old scar could be removed.  But there is a chance that the newer incision could also end up looking like this, rather than being a nice, narrow and neat incision.

Hi. With a history of 2 DVT's, I would suggest you see a hematologist to assess your level of risk for surgery, and to recommend what kind of post-op DVT prevention might be best, given your specific case.

Some surgeries, like a tummy tuck, might be unwise.

Hi.  Your hypersensitivity to light touch which is localized to a very specific area sounds like you may have a small sensory nerve that is unhappy, and causing some disordered sensations.  You could ask your surgeon about a diagnostic injection of some local anesthesia into the trigger point, and if it relieves the pain temporarily, you have confirmed the diagnosis.  Neurontin tablets may help with this as well.

Hi.  I would deflate your saline implants percutaneously in the office, and let the tissue re-drape, for at least 4 weeks.Then, I would plan a breast lift, to get your nipple back to the correct position, and to remove any residual loose skin.  Finally, you could place a more proportional gel implant, behind the muscle.It's a lot of work.  And there's no good way to do it without the lift.
