O-shot works best for decreased sensation to enhance orgasms. It also helps with mild urinary incontinence. I would recommend vaginal Estrogen or Fractional Co2 Laser like Monalisa Touch, Diva laser, or Femilift. They will give you a better result for vaginal dryness and increased lubrication.

I would try ThermiVa or Thermismooth radiofrequency to tighten and smoothen the skin. Never tried it there before but I think it would help and may avoid risky surgery in this area.

Labiaplasty is a work of art and can be modified according to your goals with an experienced surgeon. However, I would not recommend both techniques at the same time. Too many incision lines will increase the risk of separation and infection.

Generally, we only remove the skin around the clitoris without touching the nerves. I have not seen a nerve damage from clitoral hood reduction so far. However, this surgery is too close to the clitoris, so theoretically, there is always a small risk of damage with distorted anatomy or unexperienced surgeon.

It is preferred to do the Thermiva first, as heat from the radiofrequency may damage the platelets you just injected. You can do O-shot in the same session after Thermiva to promote healing, but I always do the Thermiva first.

In my opinion, you need a V-Y wedge labiaplasty with clitoral hood reduction which will tighten the labia and remove all the redundant skin. The skin edges of the labias will be the same as original but a triangular wedge will be removed from each side along with the clitoral hood folds. If you want the skin edges to be lighter, you can do a linear labiaplasty with clitoral hood reduction but this will not tighten the labias.
