Based on these photos and your complaints, I would recommend a true rhinoplasty. Liquid rhinoplasty is not a good way to support the tip. Though you can push the tip up a bit with a columellar injection, the support is not good over time. I think with a well-planned and well-executed rhinoplasty, you will be very pleased.

Can rhinoplasty be done with a bone graft rather than an implant?

My recommendation for you is a diced cartilage and fascia graft. I would not go to a bone graft as they are a bit too harsh in a situation like yours. Consider the DCF graft. I think you will like the results much better!

Your profile could seriously be incredible! The thing to do is first create a radix take-off point for your nose that is defined and clear. I would then contour the bridge with a hint of a curve...and then sculpt the tip. Get a consultation with a rhinoplasty surgeon where you like their before and afters!

Is Belladerm recommended for a dorsal camouflage graft?

My recommendation is to use autologous material. I don't know about Belladerm in the nose. But, I do know about deep temporals fascia on the dorsum. This is my recommendation.

Its difficult to say if your nose will saddle. Repairing the septal perforations that are not able to be closed can be left alone. However, the support structure needs to be evaluated for its strength. There are opportunities to strengthen the support and they may be necessary.

The methods used to repair a polybeak deformity depend on the reasons the polybeak exist. If you spend some time making the diagnosis as to why the polybeak is there, then the method to treat will become obvious. Some are due to excess thick skin. Some are due to an over shortened nose. Some are due to scar tissue. Some have a combination of these. Therefore, the way to treat the polybeak depends on the assessment. Sometimes grafts are needed. Sometimes not.

I think I would need to see you in person to decide what order. Safety in surgery is first and foremost with any operation. My initial response is to do the jaw and chin, then the brow, and finally the nose. However, it should not make too much difference if you want to approach differently.

Based on the photographs, I think you can definitely get a change in the nose that will be favorable. I am of the mindset that nose reshaping can be achieved if the patient understands what they want and can communicate that to the surgeon.
