Breasts usually stop developing by 18-21 years of age, which is why most plastic surgeons will generally recommend waiting until after that age for any cosmetic breast surgery, but you can do certain breast surgeries in younger patients. Even though the breasts are fully developed, they continue to change with age, and especially with pregnancy and breast feeding. It's important to talk to your surgeon about all these things when considering breast surgery!

This can certainly be very frustrating, for both you and your surgeon! First, I would recommend speaking with your plastic surgeon about this. You likely have scar tissue in the deeper tissue of the breast that has caused the overlying skin and scar to contract, and give your breast an abnormal shape. If this has not improved with time, then it can be removed surgically, and sometimes under just local anesthesia.

Your breasts and breast skin will continue to change (and age) over time, even with implants, but by now the implants have settled into position. It is a little challenging to tell from the photos, but you did appear to have some asymmetry before surgery, and it looks like you have the same asymmetry now after surgery. Overall, I think you have a pretty good result, and you're being too hard on yourself!

You are an excellent candidate for breast augmentation! You really do not have any breast ptosis (aka "droop"), so I don't think there is any reason that you would need a breast lift.

You can certainly maintain the slightly upward rotation of the nipple with a breast augmentation. However, it may take a while after healing from you breast implant surgery before the implant "settles" and you achieve the look you're hoping for. There are a wide variety of implant styles and sizes that may be right for you, but you may also want to look into fat transfer to the breasts.

By the appearance, you can certainly achieve upper pole fullness without a lift, but because of the position of your breast, it would appear that your breast had "fallen off" the implant, hence the term "Waterfall" deformity or "Snoopy Nose" deformity.
