Unfortunately you have been left with excess skin, so liposuction or coolscultping won’t work to remove or contract the skin significantly
The best option for you would be a brachioplasty – this would involve cutting out all of the loose skin and would leave you with a scar along the inside of your upper arm

In my experience it would be extremely likely that you would require a graft. Ultimately this is determined by your original size & your desired size plus your height and chest height, as well as your notch to nipple distance. However, 37 & 40cm are very low and I generally start considering a graft when the distance is more than 30cm. I would thoroughly recommend having a reduction without implants.  You have plenty of volume to work with & an experienced surgeon will be able to give you a great result with lifted, fuller breasts, that won’t sag over time.  All reductions (if performed correctly) come wth the bonus to a lift! Hi – thanks for your question & photos.  I would absolutely recommend explant alone.  I think it may take time for your skin to recover, but that you will get a great result.  Allow at least 6 months post op for everything to settle, if you were to consider any revision.  However I think you will be pleasantly surprised with your outcome.  All the very best
