Your imnplant needs are unique and depend on your physical attributes. Likely, you will need a larger than average implant size, but it depends also on what your starting breast tissue is like. Also, losing weight would be highly recommended because there are higher complications in patients with higher BMIs.

I would redo the lift with a small vertical pattern. This does not rely on the skin for lift and the areolas can be made smaller and smoother. In general, Benelli lifts don't do very well.

By asking if you are responsible, I'm not sure if you mean did you do something to cause it or should you have to pay to have it repaired. The larger you want your butt to be, the more fat you will need available for harvesting. We can usually find fat in areas that you didn't realize it existed, though! I don't personally like using implants because of the higher complication rate.

It is hard to say just from photos, but it may be that your implant on that side is dropping below the intended inframammary fold. See what your surgeon says. You may need to wear a strap or underwire bra to keep it from worsening.
