This occurs on buttocks due to active sebaceous gland infection. Inflammation often leaves hyperpigmentation that fades overtime. We had good results in treating the condition when it occurs, with oral antibiotics for short time, followed by Nd.Yag laser use to coagulate each spot. Then after they dry out, to improve scars we do RF Microneedling. You get less breakout and improve look. Hope this helps you. Topical Retin A use is not effective for this area.

You are on good topical care treatment. If you are having side effects from oral antibiotics, try topical antibiotic. I would check your blood test for total and free testosterone, DHEA, FSH and LH to make sure you do not have polycystic ovarian disease. Birth control pill use can help you and I would increase the dose of spironolactone also to 100mg daily. You can also look into laser or RF Microneedling treatments that helps to clear the skin. We do labiaplasty under local anesthesia only. We do not see need for IV sedation also in our practice. Cost is a concern with general anesthesia as it has to be done in surgery center or hospital. Seek more opinions in your area.

Botox or any neurotoxins are contraindicated with multiple sclerosis condition as it can weaken muscle action further. Rarely they can travel to other parts of body and weaken muscles. You can do Juvederm fillers. Check with your doctor also who is caring for MS.

Microneedling is radiofrequency energy that works over time. Look into Microneedling Intensif by Endymed that has 25 gold plated needles and you can penetrate 2.5 to 3.5 mm deep.. One can deliver more significant energy and deeper with this versus Microneedling pen. Along with PRP it produces great results. Generally results are better with at least three treatments of Microneedling one month apart and one PRP. This works well for fine lines, resurfacing and acne scars….
