It looks like you are correct in your diagnosis that the breast has fallen off of the implant. You might need a breast lift to bring them back up. Sometimes you can reopperate to bring the  implants down a bit to make the overall contour better.. your surgeon should hear your concerns.. Call them.

It is not unusual to have a rash in this distribution after surgery if your skin is allergic to the long acting skin disinfectant used at surgery. Rarely my patients require steroids to calm the rash. Most rashes resolve with time and aggressive skin washing. If it does not get better report it to your surgeon.

You will need a revision and will need to be prepared to not lift weights for months after the repair. You need  to keep the implants under your pectoral muscles because you are so thin. You will have to accept the risk of flexion deformity/ double bubble seen on flex.

I do not float my belly buttons. I believe a small vertical scar with a good button above  is a better option to allow for optimal tightening and a low horizontal tummy tuck scar.

Relaxation in the first months after face lifting is not unusual.I think that this is a very reasonable approach to retightening your neck. I perform touch ups like you are suggesting in my office and the patients are pleased.

Three weeks post op is too soon to really know if the grafting is necessary.I would think in rare instances this might be helpful. I have never used this approach for belly buttons

You seem to be a candidate for fat grafting to your buttocks with fat taken from your abdomen. Do not expect a flat abdomen with this procedure or even with the addition of a tummy tuck.  You are carrying too much weight inside your abdomen and this can only be removed with weight reduction
