In answer to your question, you have to do a bit of both. The right breast needs to be reduced a little ( only if you want) and the left nipple needs to be lifted. There is almost no way to lower a nipple that is placed too high. You should post some before pictures so that your prior level of asymmetry can be assessed as well. Hope this helps.

Looking at your shape it appears that you will require implants and a breast lift to get the best result. I recommend a thorough in person evaluation when possible. Make sure that you see a board certified plastic surgeon who has a lot of experience with cases like yours.

Based on your photos you should have a nice result with a breast lift. You may not have enough fat to add a significant volume to your breasts though. I hope this has helped.

Without feeling the area it appears that it is just an extra little area of fat. It may be some accessory breast tissue but does not appear to be so on the photo. It could probably be liposuctioned to flatten it out if you desire. Once again it needs to be examined by a Board Certified Plastic Surgeon in order to make an accurate diagnosis. Can I get a breast lift on 38DD at age 58?

If you are healthy you can have surgery at any age. 58 is not old and Breast Lifts and Reductions are performed quite frequently at that age. Make sure you see a surgeon who performs a lot of these procedures and who will guide you through the process. I hope this has helped.

How large you go depends on 2 things, your anatomy and your size desire.You appear to have a narrow breast so you may be limited with the size of implant that you can have placed. I think a 365 or 385 will be good for you. You will be a C cup for sure with those sizes. If you want to be larger than that then a 415 may work for you. To begin with, 3D imaging is not always as accurate as stated. Secondly, you should post pictures of your natural breasts and not the enhanced image breasts to get a better opinion on what can be done.That being said, yes you can go larger than 275cc. Maybe not crazy big, but definitely fuller

How I would approach your situation

First of all I would recommend losing your weight first because everything is going to change based on your weight. After your weight is stable I would do a breast lift on you with a possible liposuction of the abdomen and flanks and fat transfer to your upper brass pole. If you still want more volume after that then approximately six months afterwards I would place implants.In this way we can get you the exact size and shape you want. I hope this has helped.

From the photos that you have posted it appears that you lack some upper pole fullness and may require a slightly larger or higher profile implant but you also look like you have too much skin in the lower pole and lateral portion of your breast. I think that a revisional breast lift and implant exchange into the same sub-muscular, or dual plane pocket would be appropriate for you. Just like anything else Plastic Surgeons have differing opinions on what procedure works best. Sometimes a tummy tuck would give the best result and sometimes the patient will only need to have liposuction. It depends on how much loose skin the patient has. I recommend that you go with the Surgeon that you feel the most comfortable with, not just the cheapest one. As in all things skill and results are what matter. I hope that this has helped.

From the picture that you have posted you appear to have a significant asymmetry of your breasts. The exact cause is uncertain but you will need at least an examination and possibly and MRI to be performed. I recommend seeing a Board Certified Plastic Surgeon who has a lot of experience in revisional surgery.

Realistically, your breasts will be very difficult to get to the look that you want. In your case, in order to get you to that look I would recommend a 2 step approach. The first step would be to shape the breast with a reduction/lift type surgery. The second step, 3-6 months afterwards would be to add implants to give you the size you desire. I personally like to place the implants below the muscle. I feel it gives you a longer lasting result. If you firmly believe that you want the implants above the muscle then it seems that you have an adequate amount of breast tissue.
Can I breastfeed after a breast lift and will I lose all sensation to my areolas/nipples
You will probably have no changes in sensation of the nipple

Depending on what kind of lift is performed various changes in sensation can occur and breast feeding may be affected. Without seeing your exact level of drooping it is very hard to tell what needs to be done. I recommend seeing a Board Certified Plastic Surgeon who has performed a lot of these procedures and asking him/her all of your questions. Most probably you will be fine but weird things can always happen.
