Is it normal for my implants to still look like this 6 weeks out? What can I do?

Tranaxillary implants can sit high, however, you are still in the recovery phase. As swelling decreases and tissues relax they typically 'settle'. However, your photos seem to show implants are high. Follow up with your surgeon as they recommend.

I'm 4'11" and 95 pounds, petite. I'm currently a 34A and want to be a C cup.

I believe that for a surgeon to understand what a patient desires for their final breast size, requires a visual approach. Cup size is abitrary and I do not think it is useful. In my practice I use 3D imaging to simulate on each patients images the final look. From this and the exam, I can then make an educated recommendation regarding the actually volume (cc's) and type of implant to use.

After augmentation more than likely your breast base width would increase. Base width is an important measurement pre-operatively in selecting an implant size that is appropriate for your anatomy.

There are no hard and fast dates . If you are breast feeding I suggest waiting to have a consultation closer to when you will discontinue breast feeding. At the least I suggest patients wait 4 to 6 months from delivery to have a consultation and at least 6 months to have surgery.

I would not plan an elective cosmetic procedure for that length of time. If it is reconstructive, non elective surgery than you may have no other choice. To better answer your question it would be best to know what surgery is planned.

These include non surgical such as fillers, Ultherapy, skin care, laser etc. The surgical option is some type of facelift. Implant size is one of the more difficult decisions for patients. However, with careful planning and discussion with your surgeon you should be able to reach a nice result. Base width is important as a guide to the implant size. I personally do not have patients use external sizers and bras. I feel this is not the most useful. The vast majority of patients do best with a standard tummy tuck. Meaning the incision is placed low on the abdomen. A select few , with excess skin only in the upper part of the abdomen may be candidates for a 'reverse' tummy tuck. In this case the incision is placed under the breast crease. Based on you pictures you would likely require a standard tummy tuck.

Based on the photos you provide you likely would need a breast lift, mastopexy, in addition to implants. Your breast position and nipple position have dropped and therefore need lifted.

The necessity of a breast lift is based on both the nipple position and the overall breast position. The "pencil" test is not a good indicator. If the nipple is at or below the breast crease a lift is likely needed.

You may be a candidate to add some volume with fat grafting. However, you also need a breat lift to address your dropping/ptotic breasts.

Based on the photos you cannot determine implant position. With your pre op pictures, and my preference, I would typically place an implant below the muscle. Silicone implants provide a softer, more natural looking breast with less rippling, especially in thinner patients. You can still achieve a lot of upper breast fullness with silicone gel. The muscle does nothing to support the implant, as it relates to your last question.
