A back lift has nothing to do with an extended abdominoplasty. They are two different operations that treat two different areas. An extended abdominal plasty or trunk lift works below the waist and a back lift works above the waist.

A back lift is offered combined with a breast lift. I prefer them secondary to the tummy because they are pulling in different directions.

One of my implants never formed a capsule, how rare is this & could it form completely now?

This problem of an extruding implant is usually because of an indolent infection. An alternative also is a use of peri- prosthetic steroids, which is rarely done now. That was used in the past, but led to thinning of the lower pole.

It is possible that your surgeon was able to salvage your implant, but it is also likely that you will thin again and lose the implant for a time.

After a month or so, without an implant, it can be replaced.

Your photographs do reveal the appearance of your old from memory fold as a tight crease underneath your breast between your Rea and the new fold.

This is often the result of an excessively large implant based upon based dimension, or perhaps in your case a version of constricted breast.

There are two possible corrections. One is to downsize and do a lift which I doubt you will want. The other is a revision to release the tight crease from inside to allow the lower pole to expand. This…

Unfortunately, your photograph does not reveal the lateral aspect of your breasts. The appearance of “a wide breast“ can often be caused by excess lateral inferior breast, extending into the axilla. This can be corrected but a better photograph and in person exam would be needed for confirmation.

It appears to me that you’ve undergone a significant amount of weight loss. You have a generally slim frame. I think you do not need a Florida Lee procedure and would do fine with an extended abdominal plasty perhaps a trunk lift but I don’t have posterior views.

Liposuction is a part of every single tummy tuck I do if not not to reduce fat, at least to equalize and distribute the tension.

I was suggested multiple consultations.

My breast is saggy and grows downwards. My areolas are also huge and I want them small

Based on only a single photograph, it appears that you would do well with a breast lift procedure. You lack upper pole fullness, so an inferior flap autologous augmentation would be my recommendation. Not many surgeons do this procedure, although it has been a viable option for years.

Standard lifting will ultimately fail because the lower pole will stretch.

Thank you for your question with photographs. In my opinion, your initial surgeons recommendation or advice is incorrect. I think that a full tummy tuck should not leave you with an extra incision if done with progression tension quilting sutures, with an experienced hand during the work.
