Your photos demonstrate sagging of your breasts which needs to be addressed. It is difficult to tell from the photos you submitted how much true sagging you have. The amount of breast tissue below the crease, the position of the nipple, and the skin tone among other factors all play a role in determining the best procedure for you. You may have answered your own question. You are not really an ideal candidate for surgery with your current BMI. It does not mean you cannot have a procedure done, but the risks of surgery increase...infection, delayed healing, and blood clots. Weight loss with dieting and exercise is preferred as it is safer, requires no recovery, and it is certainly less expensive. You will likely need a tummy tuck even more after losing weight as the skin on your abdomen will deflate and droop more....

Adding a lift ads considerable risk, scars, recovery, and cost. It can be done as a second procedure if desired. It is unlikely that epicanthal folds will resolve spontaneously. Typically a minor procedure is required to lessen their appearance. It would result in small scars in the corners of the eyes, but that has to be weighed against how much the folds bother you. Scars almost always improve with time, but they are permanent.

You would need to ask your surgeon this question to know what is meant by the question. In general, it likely means that even though the skin and fat is removed from below the belly button, there may be left over fat above the belly button which may require liposuction to thin it out. Weight loss before surgery may also address the issue, and liposuction may not be needed. Discuss this with a board certified plastic surgeon to know your options.

I have seen numerous patients where a lift may of may not be required to obtain the best result. Based on the photos, you may be able to get an acceptable result with an implant only. If you are patient and willing to wait 2-3 months for the implants to drop and settle into place, then implants alone would be a good option as a lift can always be done later if needed. I always say there is a person attached to the tummy, and the surgeon needs to address the entire patient to be sure surgery will be safe. It seems you would need an extensive medical exam and clearance from both a neurologist and a cardiologist to make sure you are optimized for this type of elective surgery.

This all depends on surgeon preference. Permanent sutures will hold the repair the longest, and the risk of infection is extremely low. Dissolvable sutures are used by some, but the repair may not last. Once healed and the repair is done well, one should not have to worry about "popping" a suture.

It is difficult to tell exactly where the problem is located from the limited photos, but it appears the implant has descended down somewhat which has stretched out the bottom of the breast. Be sure to be examined by a board certified plastic surgeon to understand the issue better. In general, the treatment would be to repair the fold so the implant shifts up into its original location.

In general, breast reduction surgery for large, heavy breasts has been shown to be very effective in reducing back, neck, and shoulder pain. I may be covered by insurance, depending how much tissue is removed and your benefits. It has not been shown to affect asthma. You can transfer fat from one part of the body to another, but the results are not consistent. It is best to submit a photo with you looking straight at the camera, not looking up which will always accentuate the fat. You may benefit from a blepharoplasty.
