Would a lower eyelid lift be a good solution for sagging skin? 26 y/o
lower eyelid sagging skin
Aug 22, 2023
Hello, and thank you for your question. Some of the prominent rhytids (lines) demonstrated around your eyes with expressive smiling can be reduced with the use of neurotoxin (botox or dysport). It appears that you have some chronic malar (cheek) and possibly tear trough edema (swelling) likely secondary to restylane injections which can obstruct the lymphatic drainage system of the upper cheek and orbital region for up to 8-10 years based on recent studies. This obstruction can cause boggy...
Can lower eyelid fat repositioning damage the tear trough area?
lower eyelid fat repositioning
Aug 22, 2023
Hello and thank you for your question. Lower eyelid fat can be addressed in a number of ways. Fat can be transposed across the orbital rim to prevent hollowing in the tear trough and to replace volume deficiency along the orbital rim. However, I have found this technique provides inadequate volume. I prefer to remove the herniated fat bags and address the volume deficiency along the tear trough and orbital rim with microfat grafting to smooth the lid cheek interface. This provides more fat...
Botox for chronic migraine has made my eyebrows droop badly. Would a brow lift be suitable in this si
botox and brow ptosis
Aug 10, 2023
Hello, and thank you for your question. As we age the ligamentous attachments of the brows to the orbital bone become lax, the underlying bone loses volume, and we utilize our forehead muscle (the frontalis) to elevate our brows and clear our visual field. Botox for migraines is injected in the region of the frontalis. The frontalis is the only muscle which elevates the brows. Botox weakens this muscle so we are unable to compensate adequately to elevate the brows. If you are unable or...
Suggestion on procedure to improve my eyes?
eyelid surgery
Aug 10, 2023
Hello, and thank you for your question. It looks like you would benefit from a combination browlift and upper eyelid Blepharoplasty (lift). You may also benefit from lower lid canthopexy/plasty to tighten up the tendon at the outer margin of your lower eyelid. This will lift the outer corner of your eye. A Board Certified Plastic Surgeon will be able to fully evaluate you and determine a plan to meet your goals. Best, Dr. Brostrom
1 year post upper eyelid surgery - was enough skin removed?
upper eyelid surgery
Aug 10, 2023
Hello, and thank you for your question. A thorough examination will allow one to differentiate between thin upper eyelid skin and thicker brow skin. The brows and upper eyelids are intimately related. As one ages the ligamentous attachments of the brow become weaker, and the brow begins to drop. The initial evidence of brow ptosis is an apparent skin excess over the upper lid. I suspect you have a combination of brow ptosis (sagging/drooping) and some residual upper lid skin excess. You also...
How can I reduce my eye bag?
lower lid pigmentation
Aug 10, 2023
Hello, and thank you for your question. I don't appreciate "eye bags" from your picture but rather shadowing and hyperpigmentation (darker pigmentation). Skin over this area is very thin. Hollowing or volume deficiency in the area of the tear trough can result in shadowing and dark circles. Microfat and nanofat grafting to this area can be beneficial to improve the volume and pigmentation. Fat grafting is not entirely predictable but often does well in this area. Fat grafting in this area is...
What can be causing the bags between my eyes and cheeks and how can they be corrected?
lower lid bags
Aug 8, 2023
Hello, and thank you for your question. It is difficult to tell from your limited picture, but you appear to have prolapsed lower lid fat pads. This commonly happens as patients age, and the orbital fat begins to herniate or bulge through the orbital septum of the lower lid. This can be addressed with lower lid blepharoplasty which will reduce these prominent fat pads and tighten the muscle and skin of the lower lid resulting in a smooth lower lid-cheek transition. See a Board Certified...
