Statement: Board-certified plastic surgeon Dr. Luke J. Curtsinger treats patients of all ages from across the state of Georgia, from Savannah and Bluffton to Brunswick and Hinesville. In over 28 years of practice, he has performed more than 20,000 surgeries, all with the goal of satisfying his patients. “Nothing makes me happier than when my patients look at me with tears in their eyes because, finally, they are happy with what they see,” he says.

Dr. Curtsinger specializes in many plastic surgery procedures, including breast enhancement (breast augmentations, breast lifts, and more); facelift surgery and neck lift surgery; mommy makeovers (including tummy tucks and Brazilian butt lifts); Botox, Dysport, cosmetic fillers, and more, giving patients in the Savannah area and beyond a plethora of options for looking and feeling their best. After graduating magna cum laude from Murray State University, Dr. Curtsinger went on to receive his medical degree from the University of Louisville School of Medicine, where he earned multiple accolades and graduated in the top 20 percent of his class. After completing his general surgery residency at the University of Texas Health Science Center, he completed two fellowships—the first in wound healing at the University of Louisville and the second in plastic and reconstructive surgery at the University of Pittsburgh. He is board certified by the American Board of Plastic Surgery and is a member of the American Society of Plastic Surgeons.

Dr. Curtsinger is extremely active in the medical community. He has been an assistant professor of plastic surgery at East Carolina University, the chief of plastic surgery and director of plastic surgery education at Memorial Health University Medical Center in Savannah, Georgia, and was the 2018 president of the Georgia Medical Society. He frequently lectures about skin healing, and has authored 15 scientific papers on this subject and others. Dr. Curtsinger also co-founded the University Hospital Skin Bank in Louisville, Kentucky.

It is difficult to assess your asymmetry on the picture because of shadowing, but I think that I see some heaviness just above your nasolabial fold.  The treatment could involve some conservative liposuction along that line.  Facial aging is often a combination of sagging and volume loss, and the optimal treatment of facial aging often involves correction of both.  I am sorry that your volume restoration was less than you hoped for, but improvement is likely possible.  
What size and profile implant should I get? I want to have the fullness back and roundness like

I recommend that your present your wish pictures to a BC PS for an evaluation. Once you get an idea about the volume desired (I like for patients to try on implants), then the critical factor is that you then pick a base width that complements your goals. That is the best way to pick the profile, no matter what Marketing name (like ‘high’ profile) that the implant company has placed on that implant.

If your doctor is on vacation, there should be a mechanism to get your questions answered. Your doctor probably has coverage, either a nurse in his office, or coverage by another PS. Call the office and get your questions answered to your satisfaction.

See your PS. If this is developing in the second year, it could be signs of capsular contracture. Other possibilities include a late seroma. Radiation increases the chance of capsular contracture.

That doesn't look like a bruise.  More like a rash that could be fungal or hypersensitivity rash.  Additionally, you are now getting some dark, hyperpigmentation spots.  That can be toned down with some hydroquinone, but I think a dermatologist would be ideal to help make a proper diagnosis.

Most implants start out 'high and tight', and over the first 3-6 months they almost always drop down and forward into the breast envelope, which looks much better.   I would agree with the breast band, to help this happen.  Trans-axillary breast augmentation does seem to have less predictability on the pocket matching the fold.  Give this some time and the band and let it settle first.  
My doctor said he couldn't go any smaller than a G to D cup because it would damage to my breast

I am sorry that you did not get the size that you want.  I believe that your doctor was concerned about the blood supply to your nipples and breast flaps.  The more that these are 'thinned down', the greater chance of poor healing or necrosis.  It is challenging to take a very big G breast and make it very small.  I would give it 6-12 months, but in the mean time, you can communicate your wishes to your PS.  Some additional reduction is possible after things have settled down.

The transverse scar of breast reduction is usually placed at or near the fold, and then as the breast settles, it can migrate up or down a little.  Better too high of a transverse scar, than too low, which is more likely to migrate out of a bra or bathing suit.  I am not sure about your pain.  I dont think that the scar is 'causing it', but with the previous implant surgery followed by reduction, there are more than one possible causes.  I believe that you should evaluate this with your PS.

I have managed to do several breast lifts and reductions under local with sedation, but it is a struggle to keep the patient comfortable.  I would recommend that you go over your fears of anesthesia and the risks and benefits of both choices with your PS.  If the patient is comfortably un-aware, the surgery procedes smoothly and quickly. Give me a well planned, well controlled, general anesthetic by an experienced provider any day.   
Is a reduction from H to D with a history of keloid scarring a realistic surgery for me?

Most women find the trade offs (scarring, perioperative pain) are far and away worth the benefits (decreased back pain, neck pain, shoulder pain)You need to clarify your history of scarring with your PS.  Many patients dont have keloids, but hypertrophic scarring, which is easier to manage.  Either case, these scars can usually be managed well to make the procedure worth the risk.  
If I have a reduction after my breast lift would the areola and nipple have to be moved again?

Discuss your concerns with your PS.  The most important factor is the current position.  If it is in good position, then the nipple does not need to be moved,  Additionally, assessment of the areolar scars and areolar size could indicate the need for revision of the nipple/areola.

Dog Ears Will Settle Down

The excess tissue at the end of your incision are some bunching called 'dog ears'.  Happily, a lot of this will go away in the first 6-12 months.  Discuss optimal scar care with you PS.  This will help too.  Nipple asymmetry may be due to asymmetric swelling, and some mild asymmetry may be tolerated.  Give it 6-12 months as many issues will resolve with time, and if a revision is needed, it will be smaller and easier, and mor precise.

I believe that I have done almost every combination.  The guiding principles are that the surgeries don't go too long, and the patient can manage well post op.  For your four surgeries,I frequently stage the abdomen and thighs in one procedure, and the breasts and arms in another stage.  If the patient is too large, then the tummy and thighs need to done on different days.  The most common combination in my practice is breast and tummy on the same day.

I believe that you have over-activity of some of the untreated areas of your chin.  You can wait this our for several months and it will likely resolve, or, alternatively, the over-active areas can be calmed with more botox. I am sorry that you are having trouble.  If your injector refuses to help, you may need to seek consultation with someone else.
