I would recommend that you have a lift only if you’re comfortable with the size of your breasts. Otherwise I would consider doing an augmentation and a lift.
The location of your incision would depend ultimately on the size of implant that you have.
The bigger the implants, the less skin removal, hence the possibility of not having a vertical scar or horizontal scar on the breast.

I my opinion, you will be sadly disappointed with just a breast augmentation. The procedure indicated in you is a full breast lift with the insertion of breast implants. If you aren’t ready accept the scars and are tight with your money, then just wait.There are surgeons who prefer to do one or the other procedure first and then come back and to the other. If you have the breast lift done first and then have the augmentation, you will still have to deal with the scars of the lift. You would then come back months later and have the breast augmentation. I you do the opposite and have the augmentation done first, you will be very disillusioned with the results and then have to wait to do the lift.Understand that there are advantages of doing both at the same time… less costly, one anesthesia, one recovery. You have a clear indication of a Breast Lift… I completely disagree with whoever told you BA alone
Only by looking at your photos I can tell you a Breast implants alone will give you an unsatisfactory outcome. You have a clear breast ptosis, where you can see your nipples are starting to incline downwards. In order to gain an interesting volume with an aesthetic shape, a breast lift is also needed.
If it were a very mild ptosis, I could tell you that maybe a big implant or a periareolar lift could help you avoid a vertical scar lift. But you posted a photo lifting your arms (very good of you to think of that by the way) where you notice, not even then your breast look lifted. Hope this answers your doubt. (Luis A. Implants are just fillers. They make what you have larger. A breast lift will reshape your breast. Your nipples are very low. I am not sure you will be happy with only a breast implants. No lift and implants only
This is what I do to maximize lift with a breast implant alone:- use silicone cohesive gel implants. The more cohesive the better.
I am not sure if these are available yet in the US, but the Allergan Inspira Truform 2 implants are ideal- use high profile implants. The more projection you have, the more lift you will get – have the implants placed in the submammary pocket (under the breast itself).
You should have a pinch thickness at the top of your breast of at least 2 cmHowever, you may need more than this to achieve a perkier breast contour. (Jerome Edelstein, MD, Toronto Plastic Surgeon)
Breasts sag without lift

Breast implant with or without a lift
I would recommend a lift with a breast augmentation procedure. It appears that your nipple position is very low, and breast implants will definitely add volume but will not improve the sagging. The scars from a breast lift procedure can be minimized using good scar care after surgery (we use Scar Recovery gel from SkinMedica) which patients have been very happy with.
