There are different degrees of breast constriction/herniation and so whether the problem may be corrected via fat only depends on how severe your case is. In the more severe cases, fat injection only will not address the problem as the breast skin that is protruded would be more likely to expand, compounding the problem. Initially the breast would need to be reshaped, following this, fat grafting would be used to augment volume.

Depends on the degree to which your areola is reduced.

In general, the vertical incision bears much of the tension in performing a keyhole pattern lift. If the areola needs a large reduction then the tension on the non-pigmented skin/areola junction may be such that the scar spreads and heals in a very unsightly manner. Non-dis-solvable suture will last long enough for the skin to relax and minimize this effect, but there is nothing extraordinary about "permanent suture." While it is possible to cover your areolae with an occlusive dressing during surgery, things move around and there is a chance that the dressing may get dislodged, compromising the sterility of the field. Why take that chance unless it is unavoidable ? (for example if it is not possible to remove the piercings). I do not recommend filler in your case. Much of what needs to be done can be described as a reduction, not an augmentation so augmenting with a filling substance would not help.
As far as a closed approach, there is a place for that but the amount of work that needs to be done to refine your tip predictably is much too much to attempt through a closed approach with any precision. There are "experts" who would attempt that to save you a very inconspicuous scar but the risk in trade-off...

You have to ask yourself, if the non-surgical solutions worked, why would breast augmentation be one of the most commonly performed procedures in the US? The answer is, it wouldn't. It would breast implant companies out of business. Do you know anyone who'd had implants? Probably yes. Do you know anyone who has successfully enlarged their breasts via these other means? Don't fall for the gimmicks, save your money, and spend it on something shown to work.

There are two main issues with going through with surgery while ill with an upper respiratory or a number of other infectious processes.

I always advise patients to forget cup size because the look differs per person. You should choose the size with the sizing system at your plastic surgeon's office and go by the appearance. With that being said, 400-450 ccs will give you the desired C cup size.

Unfortunately, even if you were to undergo a breast augmentation with large implants it would not correct the issue. If you do not want to undergo bony reconstruction (which is a big procedure) I would recommend camouflage with fat grafting. Several treatments would be required to camouflage the area. After this, I would suggest a breast augmentation with 410 cc textured form stable implants.

Implants may be downsized, but that would depend on the size of the shell originally used. The fact that you already feel ripples likely means that using the same shell to downsize may worsen the issue. As far as asymmetry; you would need to provide photos. Thanks
