Unfortunately wound dehiscence (medical term for incisions coming apart) is a common complication after breast reduction surgery. It usually occurs due to excessive tension along the suture lines. Also at that area (the trifurcation point) where multiple corners of skin are brought together, has a decreased amount of blood flow which can also be a factor in wound dehiscence.

If your question is can you get an tummy tuck at the same time as a J-Pouch? Then I would think you would be hard pressed to find a plastic surgeon (and colorectal surgeon) to do that at the same time. Too many variables for complications.
But if you're just planning ahead, and thinking about doing a tummy tuck now, then there shouldn't be any contraindications to do that now and then undergo a J-pouch in the future. Don't be too concerned about the diastasis...

I think your concerns are valid. Its somewhat difficult to see from the picture, but there appears to be a small dehiscence of your wound. I agree that it is prudent for your surgeon to evaluate you and help guide you on how you should treat this wound.
