From your photos you do appear to have a component of tuberous/tubular breast, with loss of volume medially, wider areolas, wider distance between breast, lateral chest/breast fullness. Best to be assessed by a board certified plastic surgeon, reviewing your health and breast history and in person exam. However, from just your photos I would suggest considering a first stage tighten the NAC and release the constriction/ lower the IMF, and lateral chest wall defatting to reduce that…

Your photos do not demonstrate TBD. You have a NAC diameter that is a little more than the ideal proportion of 20% the width of the breast. This could be improved with a donut mastopexy. If you would like more volume you could consider a breast augmentation with fat (smaller volume) or implants (larger volume). Best to see a board certified plastic surgeon to discuss in detail.

I advise my patients who have adequate natural volume to avoid implants (need to be replaced, weight, other issues). You can get a very beautiful result with a reduction (removing some volume) auto-augmentation (repositioning breast tissue to get more upper pole fullness and narrower breasts, and a breast lift. Tuberous breast typically includes: constriction (narrow breast width), wide areola (with pseudo-herniation) and volume deficit. It appears from your photos that you have smaller breasts and minimal asymmetry in volume and position of the nipple, but not true Tuberous breasts.

Best to be assessed in person by a board certified plastic surgeon for a breast augmentation to increase the volume of your breasts +/- the NAC position on the left.

You wide areolar due to the weight of your breasts and the quality of your tissue. The combination of the two factors has led to what appears to be grade IV breast ptosis. A vertical scar “lollipop” breast reduction would lift your breasts, narrow the areola and make the breasts smaller and more proportionate to your body. I would suggest getting to your goal weight first, and discussing with a local board certified plastic surgeon.
