It has been my experience to separate the two procedures because if you're sitting a lot on your buttocks and have a tummy tuck at the same time you're going to lose a lot of the fat that's placed in your buttocks. Based on your pictures it does look like you may have some intra-abdominal fat so it would be better to lose weight before your tummy tuck surgery.

I would recommend you have the blood removed because it would put you at a higher risk for developing a capsular contracture. I have performed this in my sterile operating room under local with a blunt cannula which can save a trip to the hospital but it may take a few aspirations. Sculptra is a material that adds volume and would make you look bigger . Liposuction is a procedure that reduces fat . So the answer to your question is liposuction would thin out the area much better . But it does look like you have a lot of excess skin that would not go away especially in the lower abdomen and you may be a better candidate for a tummy tuck if you want to be perfectly tight and flat . I remove about 98% of lipomas under local anesthesia. It takes on average about 30 minutes and most patients walk in walk out and go back to work to a desk job that day.
