It appears you have lost a lot of weight and so first of all I want to congratulate you on that. Now is your time to reward yourself. I often show patients the difference of a fleur-de-lis versus a standard extended abdominoplasty. Like you , many choose not to have the vertical scar. If however , they have chosen incorrectly and find that they still have a lot of loose skin , a vertical incision can always be done down the road. Be sure to discuss this with your surgeon ahead of time....

Some advice here without seeing photographs, symmastia you will not worsen if you place implants under the muscle. The muscle limits the dissection and therefore will not worsen your symmastia. If you place on top of the muscle it always is a possible complication of over dissection. Agree with your surgeon stating that as long as he stays 5 cm apart from implant placement you should be fine. Good luck.

Congratulations on your weight-loss. From your photographs you still have some internal fat which would be beneficial to lose before your surgery. Getting that fat off first would certainly help in trying to get the best result. Trying to tighten them now would probably just lead to the sutures tearing through and you would still have a large belly bulge . In the end you have a better result with losing the weight first. You certainly could go slightly smaller if you wanted to. Your breasts seem to fit your frame well so I would recommend going slightly smaller. If you were a lot smaller a lift would be more probable. Your breasts seem to fit your frame well so I would not recommend going too small. If you were to remove implants and do fat grafting, my concern is to remember only 60% or so of the fat would survive so be careful. Good luck and talk it over with several doctors.
