It is very unlikely you caused any major damage from your fall. You may have caused some tenderness from a "pull", however; at 3 months the muscles are essentially healed. Most repairs are typically healed by 6-8 weeks post op. Tenderness from tummy tucks can last for a few months after sugery. If your tenderness continues, please contact your surgeon for follow up. 
Will coughing do any damage to my repair at this point out?

At 12 weeks post op, the repaired muscles are essentially healed. Coughing at this point, although it may be uncomfortable, will not likely cause any damage. In my practice, I allow patients to get back to abdominal exercises at 8 weeks following their tummy tucks. Like coughing, this also relies on the muscles being healed. If you have any concerns or have  pain after coughing that doesn't go away, I would bring it to the attention of your surgeon. Best regards.

It is still quite early in the process following your mole shave. I would still give it another week of healing. There is a chance that there was some residual pigmentation or colour that was deeper in the skin and was not completely removed with a shave. This may require another shave or possibly excision. I would have this reassessed by your surgeon.

If improving your back pain is your ultimate goal, then a reduction (removing breast weight) which also includes a lift would likely be the best choice. This would also fix your uneven breasts. As a result, one may see a bit of blood or yellow drainage from the open site. It is not abnormal to retain fluid following a tummy tuck procedure. The surgery is a long procedure and you are given a significant amount of fluids through your IV. This, in addition to your blood transfusion, will result in fluid retention that may take up to two weeks to “lose” following your surgery.
