There are two ways to reduce breast size that may work. First, if overweight a woman can lose weight and breasts may get smaller. But, by your numbers you are not overweight: Your BMI is about 23, which is within normal range.

Second, if someone is “flabby” with a high body fat percentage exercise–especially weight training–can build muscle and burn fat, potentially reducing the size of a woman’s breasts. It’s not possible to answer if that might apply to you.

Breast size is a personal choice. A breast reduction (which would include a lift) looks like a good option for you from your photos. Insurance coverage will depend on what your insurance company requires and how much smaller you wish to go. An in person consultation should help answer these questions.

Moderate plus or high profile, what would you suggest?

Great question that comes up all the time in my practice. In general for a 500 cc implant a high profile would usually work best. But that is based upon an average chest width (and breast base width). Moderate plus can be a great choice if the width of the implant is less that the width of a patient’s breast, even up to 500 ccs.

Will a lift be able to fix deformation?

Yours is one situation where an in-person exam is particularly important, as well as a full discussion of goals.If your main goal is to fix the indents, fat grafting alone (over 1-2 sessions) may be sufficient.  If so the advantage is that you can avoid the additional scars of a lift.If a goal is to tighten the skin and lift the breasts, then the scars of a lift may be a good option.  Seek several experienced opinions, and best of luck!Dr. Jim Romanelli

By photos you appear to be a good candidate for implants above (or below) the muscle.  One key is that your choice of implant size is smaller, and should be largely covered by your existing breast tissue.  Another advantage is that you will not have dynamic changes to the shape of your breasts when you flex your chest muscles.The link below shows a body builder who went with implants above the muscle with me.
