It would be important for you to have an examination with a local expert.    The typical implant shell usually lasts about 20 years on the higher end.   You can evaluate the implant integrity with an MRI which your surgeon will prescribe it needed.

The best answer will come after you have a consult with an expert in your area to discuss your concerns.    From the photos, you have significant ptosis which you would have seen in the past when your breasts were larger.  If you did not like the drop when the breasts were larger, then a lift would be needed.   It is very common to separate the breast lift and augmentation surgeries for cases that are not clear cut which may be an option to explore. 
Is Sientra gummy bear implants a good idea even if I have little breast tissue?

Since you have less breast tissue or tissue coverage, silicone implants are a better choice for you.   You can choose any of the 3 implants companies but Sientra has the best follow up warranty and coverage.    The incision, size and implants type will be determined during your consultation.   You should see a few experts in your area to get more educated and to find the best surgeon for your case.

Most of the cases  of ALCL cases came from ALLERGAN textured silicone implants  ( style 410).  They use a salting process to create the texturing that may have been the issue.   Most American surgeons knew the issues since 2000 and avoided using them.   Most of the cases were found in Canada, Austria and New Zealand and  have recently banned them in those countries.   
Is the feeling of engorgement normal after 17 months of implant exchange?

Usually the side that had treatments of radiation will remain tighter and less pliable around the implant.  If it is on one side only, then you may need a revision to improve the appearance.     If you are feeling it on both sides, then it is typically  less of a concern.   In either case, you should have your surgeon examine you to address your concerns.

You need to have several consultations and discussions with surgeons about what you think is a perfect shape and breast.    From your photos, I looks like you may benefit from a breast lift to reposition the nipple complex and correct the skin laxity.     You may also need a breast augmentation.   During the consultation, the surgeon could show you what is possible.

The simulation rhinoplasty is one of the tools surgeons use to help communicate what can be expected and what the surgeon feels to be aesthetic.    If you disagree, I would consult a few more experts in your area.    Plastic surgery is an art that deals with function and aesthetic principles in relation to your body part.   A rhinoplasty can be done in many variations of the same principle.  It sounds you have made a good start in your journey of finding your surgeon.

Both of theses surgeries come with a lot of post operative swelling and pain.    Both of these are relative to each patient as the level may be experienced differently.     I usually recommend 1 to 2 week of relaxing and not doing anything too strenuous.    You may feel a sense of tiredness as the body heals which can last for a month.     You will have to listen to your body as swelling will be your indicator of you pushing yourself too hard.

There are many factors that affect the need of a breast lift.   Your photos show that you may need a breast lift.   It is common to stage the surgeries and you can start with the breast augmentation.    If you are still not happy with the overall appearance that I would do stage 2 ( breast lift).

During the buttock fat transfer, liposuction will be used to contour and harvest fat for the transfer.   Liposuction in itself will cause skin laxity.   The extent will vary among patients and expectations.   There is a high chance that an abdominoplasty will be desired after liposuction that is done for the buttock fat transfer.

Restylane injections around the nose typically do not cause broken capillaries.  You should look at the pre procedure photographs to see if these are new.     The broken capillaries can be treated and removed easily with a laser.  
Will retinol make someone more sensitive to eyelash extensions adhesive?

Retninol can cause irritation or more sensitive skin which can make any facial procedures more sensitive.     The adhesive for eyelash extension is very irritating to most people's facial skin even without retinol pre treatments.
Should I go to a different doctor if my liposuction didn’t turn out great the first time?

You should discuss your issues with your surgeon so he can examine and determine what is possible.  It is not uncommon for liposuction to have irregularities which may be difficult to see during surgery when it is swollen.   However, many irregularities can be improved.
When you get a round 2 lipo/BBL will most of the fat survive since you already did 2 rounds?

The survival of fat is dependent on several factors.   The amount of fat that survives is not affected by a history of fat transfer surgeries.     If you are having trouble with your results, you should see several experts in your area to discuss a game plan.
