Because you have true sagging, which is the position of the nipple on the breast, you would need a breast lift with the breast implants. I have used all of the manufacturers’ implants. I believe the Sientra implants have the least rippling for silicone gel. Rippling is common with saline, much less common with silicone. The current silicone implants on the market are all cohesive gel- they generally maintain their shape even if ruptured. Your breasts are properly positioned so you can’t have a lift. Your breasts are built somewhat low on your chest, but this is not sagging and a lift would not correct that. With thin tissue, the gummy bear implant is a good choice because this will help to decrease rippling or being able to feel or see implant folds under the skin. Choice of the incision depends on your discussion with your plastic surgeon, but per my article in the literature, the least chance of capsular contracture is with the inframammary incision in the fold beneath the breast.
