Thank you for your question. 7 weeks from the date of your surgery is a short time interval to evaluate your final breast position. Your implants are not truly bottomed out in the sense that we, as plastic surgeons, would all agree on. Most notably you still have a defined inframammary crease with the implant above that crease. We have no data to suggest that you are not a candidate for augmentation with your condition. Thank you for your question. It would be helpful to know how far out from surgery you are, that being said your pain still sounds to be more than usual. The topical cream might well be effective for nipple sensitivity but will not have any impact below the skin surface. Most of the discomfort from augmentation surgery will be due to the muscle elevation and pocket development and as such will not be affected by a topical preparation. Thank you for your question. You do have the option to augment with no lift based on your anatomy. The decision as to whether it is the best plan is entirely dependent on your expectations for outcome. Keep in mind that a simple augmentation will not lift your breast only improve your volume and if that is in line with your expectations then you may confidently proceed. Thank you for your question. We currently do not have any data to suggest that we mount a rejection response to silicone implants. So this is not a rejection issue. Hematoma ,as you know, is a know complication of breast augmentation surgery that may occur for a number of different reasons. Thank you for your question. You are very early in your postoperative period and your implants will most definitely change position. The limiting factor is the stretch of the lower pole skin envelope to allow the expected settling. Ask your surgeon about an upper pole compression strap and some upper pole exersises to help with the settling process, as this will help. Thank you for your question. You are fortunate in that you do not have tuberous breasts. You have a modestly constricted base . You should enhance well with a proper surgical plan. The submuscular dual plane approach for your implant placement is preferred by most of us in this senario. It has more advantage than disadvantage. You pectoralis muscle has a very large origin and will not be compromised with a submuscular approach. Choose a well qualified surgeon and best of luck!

Thank you for your question. You should do very well with an augmentation with some minor adjustments to the lower pole. You do not have tubular breasts as we define them which is great. Avoid transaxillary access for the augmentation as it will compromise the lower pole correction. Best of luck!
