Additional fat grafting to the radiated skin may soften the skin and return it to its original quality but you will ufortunately need to have capsulectomies performed.
While a DIEP flap is a good option for autologous reconstruction of your breasts, I would recommend that you get a CT angiogram to map the blood vessels in your abdomen since your surgeons harvested fat from there. There are other options for reconstruction using your own tissue which include a latissimus dorsi muscle...

I am sorry to hear that you are having problems with your implant-based breast reconstruction. If you are considering revision reconstruction at this point you should seek out a plastic surgeon who does autologous reconstruction which uses your body's own tissue. There are several options depending on your body shape and size. The most common region would be from your abdominal region utilizing the skin and fat with or without muscle.
