Thanks for your history and photos.  You definitely need correction of the sagging lower pole tissue, and an implant to fill out your breasts.  As you are now thin, the implant should go under the pectoralis muscle.  I think you could get away with lower pole glandular removal and just have an incision in the crease area of your breast.  Secondary surgeries like this are more difficult.

Thanks for your question, quite an array of answers.  The vector of elevation of the neck tissue will leave excess skin in front of your ear, so the incision will definitely have to be extended in this area.  Therefore, why not take the opportunity to have the entire lower face and neck improved in harmony.  There are many ‘terms’ for this procedure, but lower face / neck-lift is probably the most appropriate.  This is not an isolated ‘mid face-lift.

Thank you for your question and photo.  I think you really have 2 options.
1) Non surgical Hyaluronic acid fillers. This area does well with these, and may last at least 1 year.
2) Surgical Fat grafting.  This is your tissue, and will last.  However it will need to be over corrected initially as up to 50% of the fat can re-absorb.  This procedure may need to be repeated depending on your results.

Thanks for your question. Fat transfer requires adequate donor site to harvest, and as you are quite slim, I’m not sure that enough fat could be harvested to help with your issue, plus, it usually needs to be repeated.  From your photograph, it appears the the skin tension lines you have are secondary to your massive implants. Often with implants that are large the capsule around the implant will form linear tether lines that you demonstrate in the upper pole of the breast.

From your photos, it is difficult to find a lot wrong with your nose. A live consultation is always more revealing.  You should be very clear with your surgeon what it is about your nose that you don’t like so your concerns can be addressed surgically.  Most tip rhinoplasties are performed through an ‘open’ approach resulting in a small scar across the columella of your nose the part between your nostrils.  Be sure to find a board certified Plastic Surgeon that has experience in rh…

The majority of your aging appears to be in the neck area, which is very common.  However, you also have early jowl formation.  A traditional Facelift with deep tissue tightening as well – SMAS layer will address both of these issues.  A mini facelift will not give you the desired results.  Please consult with your plastic surgeon.

Thank you for your photos and question. The relationship between the dorsal hump projection of your nose and size / shape of tip need to be in harmony.  Only reducing your dorsal hump would probably put your nose out of balance, and yes the tip may look more prominent.  A better solution would be to have both the dorsal bridge reduced and the tip refined to look less round.  This is best achieved through an ‘open’ approach.  Your surgeon should be able to discuss this with you.
