. . . for a revision facelift (now that you are 6 months post-op, and most swelling should be resolved). Don’t have a full set of photos to assess and not sure what exact approach your surgeon initially used, but a proper two-plane (skin and SMAS underlying) facelift should be able to rectify much of these problems (even with a pre-existing stronger jawline). . . . but is difficult to predict. A larger implant may indeed stretch the nipple (but does so indirectly – if that makes sense – especially if it is placed submuscularly, which is usually recommended to minimize the potential for capsular contracture or peripheral implant-border “show”). I’m not as much an expert on hormone therapy, but that may do more for the nipple changes you may be seeking.

You’re a bit young for that at present, but it appears you have a hereditary relative skin excess developing, and while fillers (as suggested below) may be helpful in the short term, I have done surgery (blepharoplasty/skin removal) on people in their young 30’s for similar issues, with good results; just make sure you see a board-certified surgeon with a lot of experience at that time.

My doctor uses tumescent Lipo but closes the incisions instead of leaving them open for draining.

I routinely close my liposuction incisions (although don’t really perform “superwet” technique, moreso 1:1 ratio of fluid in:lipoaspirate out) with a simple buried suture, which allows some egress but also probably limits infection potential (as stated below) and allows the incisions to look nicer, ultimately. Part of this tumescent fluid infiltrated is suctioned back out, part of it leaks out (a majority during the case) and part is resorbed by the body.

I do believe a scar excision with a linear closure (generally 3:1 length:width of presenting problem, to keep it flat) would be less noticeable, if performed well and protected post-operatively. Should be able to be done under local anesthesia only, and I would recommend taping/limiting activity (and possibly wrapping if significantly swollen) for at least 2-4 weeks afterwards.

Is a beautiful thigh lift result possible?

Thighs are probably the most challenging cases . . .

. . . almost across the board, in the sense that the skin is rarely ideal, and you’re always fighting gravity post-operatively. Yes, nice improvements can be made (and I think there is a role for early lymphatic massage by a certified professional, especially for more difficult cases), but the skin tone/quality – especially after age 30 – is probably the most limiting factor, so it’s important to be realistic in this regard before making a substantial investment.

. . . in that patients (especially in the summer) can get more sun than they think through thinner clothes (and non-UV-protected bathing suits), even “under there”. Probably won’t be as necessary after several months/when fall arrives, and most scarring is determined by genetics/”tincture of time” anyway. Hope this helps!

I would estimate this could take at least 3-6 months (possibly longer) to resolve, and certainly no potential interventions should be planned before this time (but of course stay in touch with your surgeon for reassessments as she sees fit . . . )

1 day after tear trough filler it seems very swollen, bruised and like there are bags underneath.

You could possibly do a staged procedure to “deflate” the arms with liposuction (natural weight loss better, in that you wouldn’t be potentially disturbing your blood supply), but either way a skin problem will remain, and would be best-addressed by a formal arm lift with a lengthwise incision on the inside/underside (but not backside) of the arm, extending to the elbow. . . . with just this one photo, but looks OK overall, as some “new normal” tightness is certainly to be expected, and will relax (sometimes a bit too much . . . ) Also, that positioning and any platysmal “flexing” may accentuate what you’re seeing. Time is on your side with these things; better to assess closer to 3 months out at minimum, in a neutral position, and in comparison to your pre-op photos – my guess is that you’ll be pleased (and that “turkey neck” definitely will be gone)!)
